Annual Scientific Meeting, Belfast; November 2000. General Anaesthesia Vs Total Intra Venous Anaesthesia(TIVA) Comparison of recovery following Day Case Surgery Dr. S. VijayaraghavanBishop Auckland A total of 220 patients who underwent various surgical procedures as day case were included in this survey. 121 patients had general anaesthesia, propofol induction, Fentanyl or Alfentanil,O2, N2O & Sevoflurane, muscle relaxant if needed. 99 patients had TIVA, propofol & Remifentanil infusions, O2 enriched air, muscle relaxant if needed. All received local infiltration, NSAID if no contra indication, Tramadol. All the patients were asked to fill up a questionnaire prior to their discharge & were followed up over the phone by the same day ward nurse who looked after them about their pain, analgesics, post operative nausea & vomiting(PONV), & minor complications like head ache, hang over & after taste 24 & 72 hours later. TIVA group had more female patients, otherwise no significant difference in the age, the length of stay in the recovery or in the day ward. In TIVA group more number of patients received preoperatively NSAID (43.9% vs 14.9%). Otherwise the analgesic requirements up to 72 hours. Only 2% of the patients suffered severe pain in both the groups. TIVA patients suffer the same amount of pain as that of GA. None of the TIVA patients received anti emetic (35% in GA group). Incidence of nausea is 10.1%(17.9% in GA). 6.1% of the patient had vomiting (8.9% in GA). TIVA patients suffered less headache, hangover, nausea & after taste. In summary TIVA patients suffer the same amount of pain as that of GA. They suffer less PONV & the requirement of anti emetic is less & incidence of minor complications is less. Hence it reduces the overall cost of the operation. |
